i <br /> West Side Market <br /> Site Health and Safety Plan <br /> 2/26/2018 <br /> Page 4 of 9 <br /> CVE DEMOLITION GENERAL HEALTH & SAFETY PLAN <br /> GENERAL SAFETY MEASURES <br /> When working in the area of an excavation or heavy equipment activity, access to the project site shall be <br /> limited by the exclusion zone. The exclusion zone shall include an area at least 20 feet from the <br /> demolition and associated equipment. Personnel shall not be allowed in the exclusion zone without <br /> appropriate personal protective equipment. <br /> All CVE Demolition personnel, subcontractors, and consultants shall adhere to the provisions of the CVE <br /> Demolition Site Safety Plan including all applicable elements of the CVE Demolition Illness and Injury <br /> Prevention Program (IIPP). All of the above personnel who participate in fieldwork at the site will attend <br /> a safety indoctrination provided by the CVE Demolition Site Safety Officer. The CVE Demolition Site <br /> Safety Plan is maintained with the Work Plan. <br /> Copies of the Work Plan and Health and Safety Plan shall be available to all working units of CVE <br /> Demolition, its subcontractors, and consultants. <br /> CVE Demolition will provide sanitation and washing facilities within the work area. Facilities shall be <br /> maintained and cleaned regularly. <br /> SITE SAFETY OFFICER RESPONSIBILITIES <br /> The Site Safety Officer shall monitor all activities of equipment and personnel at the project site. Site <br /> Safety Officer' s responsibilities will include the following: <br /> 1 . Limiting access to the exclusion zone to authorized personnel <br /> 2. Providing a written log of on-site activities and events. <br /> 3 . Notification of regulatory agencies during emergency procedures. <br /> 4. Enforcement of contaminant exposure site safety precautions. <br /> 5 . Documenting all injuries and safety incidents. <br /> SKIN/CLOTHES PROTECTION <br /> Boots will be worn during all activities. <br /> Hard hats will be worn during all activities. <br /> Rubber boots, rubber gloves, Tyvek suits, face shields, and safety glasses shall be worn when working in <br /> an area formerly used to fuels or oils. <br /> EYE/EAR PROTECTION <br /> Safety glasses will be worn at all times while in the exclusion zone. <br /> Hearing protection will be worn during all motorized activities. <br /> VEHICLE MAINTENANCE AND SAFETY <br /> All CVE Demolition vehicles used on site during the course of work shall be equipped with a first aid kit <br /> and a dry chemical fire extinguisher (10 BC minimum rating). <br /> All cabs are to be kept free of all non-essential items and all loose items secured. <br />
