i <br /> West Side Market <br /> Site Health and Safety Plan <br /> 2/26/2018 <br /> Page 5 of 9 <br /> The rated load capacity of a vehicle is not to be exceeded at any time or under any circumstances. <br /> Unsafe equipment shall be replaced immediately. Other equipment will be safely stored and secured when <br /> not in use. <br /> The Site Health & Safety Officer shall review with all personnel and subcontractors the equipment <br /> inspection checklist for company vehicles. The Site Health & Safety Officer shall also confirm that the <br /> equipment operator has performed a pre-startup safety inspection/observation prior to the start of work <br /> and that adequate water is available at the rig for emergency decontamination and washing in case a . <br /> worker is exposed to contaminated water or soil. Training certification records for personnel operating <br /> construction equipment shall be maintained for inspection on site. <br /> Field vehicles shall park in an area that is a safe distance from operating equipment and vehicle traffic <br /> with the location approved by the Site Safety Officer. Care must be given when approaching heavy <br /> equipment. Personnel on foot should approach from a direction facing the operator and not a blind side. <br /> Eye-to-eye contact should be made before proceeding within the working limits of the operator and their <br /> heavy equipment. <br /> OVERHEAD HAZARDS <br /> Overhead hazards include utility lines and trees. Prior to performing work requiring the operation of <br /> equipment above eye level, the evaluation of overhead hazards shall be made. Equipment shall be located <br /> to maximize the clearance from overhead hazards (minimum 10 feet clearance). <br /> UNDERGROUND UTILITIES <br /> CVE Demolition personnel have contacted Underground Service Alert prior to drilling or excavation <br /> activities. The property owner is responsible for marking the location of their own underground utilities. <br /> The approximate location of underground utilities shall be marked on the existing ground surface. A <br /> minimum of 3 feet clearance shall be given to marked utilities. If drilling or excavation near utilities is <br /> required, hand excavation and probing shall be performed in 64nch intervals prior to drilling or <br /> excavating activities. <br /> MOVING AND ROTATING MACHINERY <br /> Drilling activities involve the use of large, heavy, moving, and rotating equipment. Loose fitting clothing <br /> that can be caught or snagged should be avoided. Working gloves should be worn while working with <br /> moving metal parts and equipment. <br /> PHYSICAL HAZARDS <br /> CONFINED SPACES/EXCAVATIONS <br /> A space evaluation/determination shall be performed prior to entry into a confined space. CVE <br /> Demolition employees are not authorized to enter confined spaces without prior written approval from <br /> CVE Demolition' s Corporate Safety Manager or designee. Excavations can be considered confined <br /> spaces. Employees are not authorized to enter excavations deeper than 5 feet without proper authorization <br /> and without proper shoring of excavation sides approved by a geotechnical engineer. Samples of <br /> excavations should be taken from material in the excavator bucket after it is brought to ground surface. <br /> Proper barricades shall be placed around open excavations when work is not in progress. <br />
