The incorrect and <br />missing information included the following from the inspection reports: <br />-Section V was left blank. <br />- Section VI was blank and unsigned by owner/operator. <br />-The question regarding UDC sensor locations was left blank. <br />-Tank and line tightness testing was left blank. <br />During the visual inspection, the designated operator shall check that all required testing and maintenance for the UST <br />system have been completed and shall check all required items on the report. Ensure that designated operators <br />performing visual inspections at this facility are including all of the required information on the reports. <br />This is a minor violation. <br />320 <br />CCR 2635, 2637.2, 2665, 2712 Failure to comply with the installation, inspection, or record keeping of the overfill <br />prevention. <br />-The 87 mechanical flappers used as the overfill prevention system was marked as a failure on the 10/3/18 Overfill <br />Prevention Equipment Inspection Report. <br />-Calculations included in the 10/3/19 inspection report do not match the tank charts on site. <br />-A permit has been applied for on 4/8/19 for replacement of drop tubes and is not yet approved. <br />All UST systems must be equipped with an approved, functional overfill prevention device at all times during fuel <br />deliveries. The failed components must be repaired or replaced by a properly licensed, trained, and certified contractor <br />under permit and inspection of the EHD. If the failed components can't be replaced immediately, there is a possibility <br />that the 87 and diesel UST systems may be red tagged to prevent fuel inputs. <br />This is a Class II violation. <br />Overall Inspection Comments: <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, by June 8, 2019. <br />Starting September 1, 2018, all in -office CERS help will be provided at EHD hourly rate ($152).
