You will need to work with this person to secure the permit. <br /> If it truly is a new site, that is NOT on our active site list with a current billing party <br /> assigned or if there is a change to the billing party, then in addition to the <br /> requirements above, a Master File Record form (MFR) and a pre-payment of$345 <br /> is also due before a permit can be issued.
