Notification shall be provided to the CUPA at least 48 hours in advance of testing. The EHD will <br /> not accept these results and testing must be rescheduled with a 48-hour notification. Immediately schedule these <br /> tests and provide 48 hours notification to the EHD. <br /> This is a minor violation. <br /> 113 CCR 2638(c), 2638(d) Failure to submit"Monitoring System Certification Form"within 30 days of completion of the <br /> test. <br /> Annual monitoring system certification, leak detector testing, and spill container testing was performed on <br /> 10/17/2018 and a test report was submitted on 2/07/2019, 3 months late. A copy of the test report must be <br /> submitted within 30 days of the tests. Take all necessary precautions to ensure testing and submittal of test reports <br /> are performed in a timely manner. <br /> This is a minor violation. <br /> 124 CCR 2637.2(d&e), 2665(a&b)"Overfill Prevention Equipment Inspection Report Form" not submitted within 30 days <br /> of inspection. <br /> Overfill prevention testing was performed on 12/27/2018 and a test report was not submitted to EHD. A copy of the <br /> test report must be submitted within 30 days of the tests. Take all necessary precautions to ensure testing and <br /> submittal of test reports are performed in a timely manner. <br /> This is a minor violation. <br /> 313 HSC 25290.1, 25290.2, 25291 Failure to construct, operate, and maintain primary containment as product-tight. <br /> Oily liquid was observed in UDCs 7/8, 11/12, and 13/14, indicating a leak in the primary piping. All primary <br /> containment for the UST system must be tight. Immediately have a properly licensed, trained, and certified <br /> contractor repair or replace the failed component under permit and inspection of the EHD. <br /> This is a minor violation. <br /> 314 HSC 25290.1, 25290.2, 25291 Failed to have secondary containment or secondary containment not tight. <br /> Brine was found in the bottom of the diesel STP sump, 87 STP sump,91 STP sump, diesel fill sump,87 fill sump, <br /> and 91 fill sump, indicating a leak in the secondary containment. Secondary containment shall be impervious to the <br /> liquid and vapor of the substance contained and constructed to prevent structural weakening as a result of contact <br /> with any hazardous substances released from the primary containment.
