On the 11/21/18 designated operator(DO)inspection report under -Section III The DO failed to add a description to all the components marked N/A or N from section VII through XI. The DO failed to conduct the inspection within 30 days. The october inspection was conducted on 10/18/18 and the november inspection was conducted on 11/21/18. 32 days apart. On the 12/12/18 designated operator(DO)inspection report under -Section III The DO failed to add a description to all the components marked N/A or N from section VII through XI. -Section X The DO failed to mar YES, NO or N/A under the question"Has the line tightness testing been completed within the required timeframes?"and"Has the tank tightness testing been completed within the required timeframes?" On the 1/19/19 designated operator(DO)inspection report under The alarm history was printed out on 1/11/19 and the DO report stated that the inspection was conducted on 1/19/19. -Section X The DO failed to mar YES, NO or N/A under the question"Has the line tightness testing been completed within the required timeframes?"and"Has the tank tightness testing been completed within the required timeframes?" During the designated operator inspection,the designated operator shall review the alarm history for the previous month, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing inspections at least once every 30 days at this facility and are including all of the required information on the reports.
