SAN J O A R N Environmental Health Department <br /> -:a U ;� i _# �V AMENDED <br /> 10/29/2019 <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> ARCO CHEROKEE GAS&MART* 900 S CHEROKEE LN, LODI October 24, 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 301 HSC 25292.1(a)UST system is not operated to prevent unauthorized release, including spills and overfills. <br /> The fuel filters under the dispensers are not located to allow drips and leaks to drain back into the UDC. The <br /> underground storage tank system shall be operated to prevent unauthorized releases. Immediately provide <br /> approved secondary containment for the filters under permit and inspection by the EHD. <br /> This is a Class II violation. <br /> 306 CCR 2630(d), 2641(a) Monitoring equipment is not installed or maintained to detect a leak at the earliest <br /> opportunity. <br /> The diesel STP sump sensor was documented on alarm printouts as going into alarm on 11/29/18 and the alarm did <br /> not clear until 12/13/18. No paperwork was found on site for actions taken to address the alarm prior to 12/13/18. <br /> All monitoring equipment shall be maintained to be able to detect a leak at the earliest possible opportunity. Ensure <br /> that all monitoring equipment is maintained and located to detect a leak at the earliest opportunity at all times. <br /> This is a Class II violation. <br /> 314 HSC 25290.1,25290.2, 25291 Failed to have secondary containment or secondary containment not tight. <br /> The fuel filters under the dispensers are not located to allow drips and leaks to drain back into the UDC. Secondary <br /> containment shall be provided for all single-walled UST components. Immediately provide approved secondary <br /> containment for the filters under permit and inspection by the EHD. <br /> This is a Class II violation. <br /> 320 CCR 2635, 2637.2, 2665, 2712 Failure to comply with the installation, inspection, or record keeping of the overfill <br /> prevention. <br /> The mechanical flapper used as the overfill prevention system on the diesel UST was listed as a failure on the <br /> 1/11/19 overfill prevention equipment inspection and is not currently functional. All UST systems must be equipped <br /> with an approved,functional overfill prevention device at all times during delivery operations. Immediately <br /> discontinue deposition of petroleum into this tank until the component is repaired or replaced by a properly licensed, <br /> trained, and certified contractor under permit and inspection of the EHD. If the failed component can't be replaced <br /> immediately,there is a possibility that the diesel UST system may be red tagged to prevent fuel inputs. <br /> Overfill prevention testing was conducted on 1/11/19. Overfill prevention equipment inspections were required to be <br /> conducted by October 13, 2018, and every 36 months thereafter, and within 30 days of completion of a repair. <br /> Ensure all future testing is conducted within required time frames. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> An inspection checklist was provided to the facility operator on the day of inspection. The EHD has written the <br /> complete report on October 29, 2019, which replaces the initial checklist. Removed violation#210.
