No CERS submittal has been made for the new owner. Permits may be transferred to a new underground storage tank owner if a complete submittal is made to the California Environmental Reporting System within 30 days of the change in ownership. Immediately complete and submit all required CERS documents for review by the EHD. This is a Class II violation. 209 CCR 2716(a-e)Failure to conduct the designated UST operator visual inspection at least once every 30 days. The designated operator failed to document all the alarms from the attached alarm history on the 4/11/19 designated operator inspection report and failed to check that they were responded to appropriately. The designated operator documented the 4/10/19 alarm but incorrectly stated it was a false alarm caused by maintenance work conducted on 4/11/19 a day after the alarm occurred. During the inspection,the designated operator shall review the alarm history since the previous inspection, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the inspection report. Ensure that designated operators performing inspections at this facility are including all of the required information on the reports. The designated operator inspection for the following months were performed more than 30 days after the previous inspection: - Initial inspection on 8/4/19, no inspection has been conducted as of 10/24/19 (81 days) - Initial inspection report dated 7/5/19, alarm print out was dated to 7/3/19, next inspection report dated 8/4/19, alarm print out dated 8/5/19(31-33 days) The designated operator visual inspections shall be conducted within 30 days or less of the previous inspection. Ensure that all designated operator inspections are performed within the required time frames. The designated operator failed to check and note all required items on the visual inspection reports.The incorrect and missing information included the following from the inspection reports: -No alarm print out was attached to the March report -The alarm print out did not cover 12/13/18-12/17/18 -Section V was left blank -Tank and line tightness testing information was left blank -Section VII was left blank -NAs were not included in Section III -Several alarm histories were missing dates and headers and appeared to contain different information than other attached alarm histories -The report states that sump sensors were properly located but did not list any inspected sumps -According to Mr. Manish Pawha,the owner/operator signature in Section VI is dated with the date of the inspection and not of the date it was signed. During the visual inspection,the designated operator shall check that all required testing and maintenance for the UST system have been completed and shall check all required items on the report. Ensure that designated operators performing visual inspections at this facility are including all of the required information on the reports. This is a Class II violation.
