The Facility Employee Training Certificate dated 10/25/18 contained no CERS ID, and the employee Harbhajan Shergill "date of assuming responsibility as a facility employee".All site employees must be trained prior to taking responsibility as a facility employee if after October 1, 2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee Training Certificate shall contain accurate information for all employees trained. Update the Facility Employee Training Certificate to contain the accurate information and submit a copy of the updated form to the EHD. This is a minor violation. 206 CCR 2712 Failure to comply with UST operating permit conditions. A designated operator inspection was performed on 11/4/2018 and the next inspection was performed on 12/28/18 , 24 days later. The designated operator visual inspections shall be conducted within 30 days or less of the previous inspection. Ensure that all designated operator inspections are performed within the required time frames. This is a minor violation. 209 CCR 2716(a-e) Failure to conduct the designated UST operator visual inspection at least once every 30 days. The designated operator failed to check and note all required items on the visual inspection reports. -The 10/31/18,11/4/18, 12/28/18, 1/25/19, 2/19/19 and 6/5/19 did not include the CERS ID and section VI left blank. -The 3/20/19 and 4/15/19 did not include the CERS ID. -The 5/6/19 inspection report did not include the CERS ID and the owner/operator signed on 5/9/19 more than 48hours late. Ensure that all designated operator inspections are performed within the required time frames. During the visual inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and shall check all required items on the report. Ensure that designated operators performing visual inspections at this facility are including all of the required information on the reports. This is a minor violation.
