SAN JOAQUIN COUNTY <br />ENVIRONMENTAL HEALTH DEPARTMENT <br />600 East Main Street, Stockton, California 95202-3029 <br />Telephone: (209) 468-3420 Fax: (209) 468-3433 Web: www.s'gov.org/ehd/unitiii.html <br />CONTINUATION FORM <br />Page: 7 of 9 <br />OFFICIAL INSPECTION REPORT <br />Date: 09/21/11 <br />Facility Address: 3250 W Grant Line Rd., Tracy <br />Program: HW <br />SUMMARY OF VIOLATIONS <br />CLASS I, CLASS II, or MINOR -Notice to Comply) <br />Violation 204 -Disposed or caused disposal of Hazardous Waste (HW) at an unauthorized point 25189.5(a) CA <br />Health & Safety Code (HSC). <br />At time of inspection: <br />According to pharmacist Nhan Duong all empty "fill" containers including Coumadin and Warfarin are thrown in the trash. <br />The pharmacy does not have a hazardous waste program at this time. <br />Immediately cease disposal of hazardous waste at an unauthorized point. <br />Violation 605 -Hazardous waste labeling 66262.34(f) CCR. <br />-1 red bucket was missing the contents and the start accumulation date <br />-1 red bucket was missing the physical state <br />-1 red bucket was missing the start accumulation date <br />-4 white buckets had the hazardous property "other" marked but did not indicate the HP <br />-3 white bucket was missing the start accumulation date and had 'other" marked for the <br />hazardous property. <br />-1 white bucket had the hazardous property marked as "other" and had the contents marked as motor oil. The <br />container in the bucket was Kirkland Ultra Laundry Detergent. <br />-1 white bucket had the hazardous property other" and had the contents marked as Nicorette. Inside the bucket was <br />nicorette gum and a bag full of wipes/towelettes. <br />-1 white bucket did not have a hazardous property marked. <br />The following hazardous wastes were sitting on top of hazardous waste buckets. RTV employees had not had a chance to <br />put in hazardous waste containers. <br />-2 bags of baking soda in a ziplock baggie <br />-1 gallon jug and 1 spray bottle of cleaning product in a ziplock baggie <br />All hazardous waste containers shall be marked with the following information: <br />1. the words "Hazardous Waste" <br />2. name and address of generator <br />3. hazardous properties <br />4. physical state <br />5. composition (contents) <br />6. accumulation start date <br />Immediately label these containers and ensure that all containers are marked with all the required <br />information. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />EHD Inspector: <br />Michelle Henry (209) 953-7699 <br />Received By: <br />- <br />5t-Arn, <br />Title: <br />EHD 23-02-003 Rev 07/12/11 CONTINUATION FORM <br />
