There must be at least one emergency coordinator on site or on call to coordinate emergency response measures, and the following information must be posted by a phone: the name and phone number of the emergency coordinator; location of fire extinguishers, spill control equipment, and if present, fire alarm; and the phone number of the fire department, unless the facility has a direct alarm. Immediately appoint an emergency coordinator and post the required information by a phone. A form is provided that can be used for this purpose. Submit proof of correction to the EHD by November 8, 2011. Violation 703 -Employee training 66262.34(d)(2) CCR. At the time of inspection: It could not be demonstrated (not necessarily documented) that employees who handle hazardous waste were properly trained. In the warehouse: Maintenance employees are expected to respond to spills on the floor which could include hazardous waste spills. It was unclear if maintenance workers had been trained to respond to hazardous waste spills and cleanup. It was not demonstrated during the inspection that all employees who were handling, labeling, and storing hazardous waste had a clear understanding of hazardous waste procedures. At the fuel island: According to the signed training titled " Handling Hazardous Waste at Costco Gas Stations in California" and gas station supervisor the last known training was February 2010. The generator must ensure that all employees who handle hazardous waste are thoroughly familiar with proper waste handling and emergency procedures. Immediately provide training to all employees who handle hazardous waste and submit a copy of the training records, including training material to the EHD by November 8, 2011. Violation 804 -UW lamp Storage 66273.33(b) CCR. Violation 808 -UW labeling 66273.34 CCR. Violation 809 -Stored UW onsite for more than 1 year 66273.35 CCR. At time of inspection: 42-4 ft bulbs and 4 -8ft bulbs were stored in a box in a maintenance room open, and unlabeled. UW lamps must be stored in a closed container, in a manner to prevent breakage. Universal Waste must be labeled with the words "Universal Waste" and the type of waste, i.e. lamps. It could not be demonstrated how long this UW had been onsite. Immediately have this UW removed and provide this office with a copy of the shipping record/receipt by November 8, 2011. Immediately properly store and label all UW. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
