A form is <br />provided that can be used for this purpose. Submit proof of correction to the EHD by November 8, 2011. <br />Violation 703 -Employee training 66262.34(d)(2) CCR. <br />At the time of inspection: <br />It could not be demonstrated (not necessarily documented) that employees who handle hazardous waste were properly <br />trained. <br />In the warehouse: Maintenance employees are expected to respond to spills on the floor which could include hazardous <br />waste spills. It was unclear if maintenance workers had been trained to respond to hazardous waste spills and cleanup. It <br />was not demonstrated during the inspection that all employees who were handling, labeling, and storing hazardous waste <br />had a clear understanding of hazardous waste procedures. <br />At the fuel island: According to the signed training titled " Handling Hazardous Waste at Costco Gas Stations in California" <br />and gas station supervisor the last known training was February 2010. The generator must ensure that all employees who <br />handle hazardous waste are thoroughly familiar with proper waste handling and emergency procedures. <br />Immediately provide training to all employees who handle hazardous waste and submit a copy of the training <br />records, including training material to the EHD by November 8, 2011. <br />Violation 804 -UW lamp Storage 66273.33(b) CCR. <br />Violation 808 -UW labeling 66273.34 CCR. <br />Violation 809 -Stored UW onsite for more than 1 year 66273.35 CCR. <br />At time of inspection: <br />42-4 ft bulbs and 4 -8ft bulbs were stored in a box in a maintenance room open, and unlabeled. UW lamps must be stored <br />in a closed container, in a manner to prevent breakage. Universal Waste must be labeled with the words "Universal Waste" <br />and the type of waste, i.e. lamps. It could not be demonstrated how long this UW had been onsite. <br />Immediately have this UW removed and provide this office with a copy of the shipping record/receipt by <br />November 8, 2011.
