Furthermore, I am also providing you the additional information you requested for the open violations. Violation#101-This violation Is still open. To this date the City has not received information that the identification number has been re-activated. Qfe4n Violation #105-Completed and closed out. Violation#106, 107, & 108- The City has hired a consultant to address these violations and assist the City with modifying its emergency response contingency plan. o , C�—cct7 �- Violation #110 and 111-The City has hired a consultant to review the training as part of the emergency response plan review, and assist the City with developing and implementing the required training. " I"t,�-- Violation #115 & 116-Completed and closed out. /Violation #202-A response was provided for this violation and the only outstanding item to be addressed was to provide a legible copy of the signed manifest. The City now has an original copy of the manifest for May 25,2016 and a copy of this manifest is attached. Violation#204-A response was provided for the violation, and originally the City was in the process of applying for a permit to treat hazardous waste. However, the City has decided to make modifications to its water treatment process so the City does not have to treat a hazardous waste. Modifications are currently taking place to the treatment facility and modifications shall be completed in by the end of November 2016. More detail as to the actual modifications and photo's will be provided to EHD once completed.
