APPENDIX B - INCIDENT CLASSIFICATION GUIDELINES <br /> The following chart sets forth the three classifications of incidents that can be expected at the LAWFT Coles Levee Operation. <br /> Level of <br /> General Criteria First Responder Notification <br /> Emergency <br /> 1. Small HAZMAT spills confined to containment. Emergency Coordinator <br /> 2. Valve or pump packing release or small gas leak <br /> Minor 3. First Aid, Medical Treatment, or Near Miss <br /> First Person <br /> 4. Fire: Beginning stages; can stop release and able to extinguish <br /> On-Scene <br /> On-Site Incident with one 30 Ib. fire extinguisher. <br /> 5. PRV release <br /> 1. Fire unable to extinguish with one 30 Ib. ABC extinguisher. 1. Emergency Coordinator <br /> 2. Medical emergency to hospital, 9-1-1 call type accident. <br /> Serious 3. Earthquake with damage. 2. 9-1-1 as appropriate <br /> 4. Flood. First Person 3. Emergency Coordinator (or <br /> On-Site Incident s. Sustained release from tanks or bin that does not appear to On-Scene alternate) to Notify <br /> with Potential Off compromise public safety.
