State of California Department of Health Services <br /> Memorandum <br /> To Jeff Van Slooten Date : June 25, 1987 <br /> Subject: Site Safety Plan <br /> from Work Plan for <br /> McCormick and Baxter <br /> From Pamela Murcel�1 `Hare, CIH <br /> Associate Industrial Hygienist <br /> I have reviewed the subject plan for health and safety considerations. Please <br /> find my recommendations for revising this plan detailed below. <br /> In general, the plan is deficient in, the following areas: <br /> -- Training requirements - in particular daily safety briefings and training <br /> which addresses the specific hazards of this pro- <br /> ject. <br /> -- Medical surveillance - any needs specific to the hazards of this project. <br /> -- Hazard evaluation - does not address heat stress and does not adequately <br /> address noise; heat stress due to protective equip- <br /> ment and climate will be a serious hazard during this <br /> project; excessive noise exposures are likely during <br /> mud rotary drilling operations. <br /> -- Exposure monitoring - need a plan covering personal monitoring for heat <br /> stress, noise, lead, arsenic, chromium VI, and or- <br /> ganic vapor contaminants. <br /> -- Contingency plan - only addresses injury; potential for other emergencies <br /> which need to be addressed. <br /> -- Documentation - a record of field activity is provided and preparation of <br /> accident reports is addressed; other items that need to <br /> be included are exposure monitoring records, respirator <br /> fit-test records, sign-up sheets for daily briefings and <br /> other training, sign-off sheet for workers acknowledging <br /> site safety plan, etc. <br /> Specific comments on items covered by the plan are as follows: <br /> 1. page C-1, Overall Hazard Summary <br /> Delete the "x" in "low"; the text of the plan indicates an overall <br /> hazard of moderate with which I concur due to the active status of <br /> the facility, and the potential hazards associated with the contami- <br /> nants and the contractor's planned activities. <br />
