Jeff Van Slooten
Page 2
2. page C-2, Overall Hazard Level
Change dust suppression techniques "should be" used to "will be" used
and describe the procedure.
3. page C-4, Site Organization
Need reference to page number where site map/sketch can be found;
should provide a sketch which shows proposed work zones delineated
based on the sampling locations.
4. page C-4, Site Personnel
Health and safety responsibilities of t&am members needs to be ad-
dressed. Statement regarding training does not seem consistent with
information provided under "responsibility". Information implies
no one is level C trained.
5. page C-4, Level of Protection
This project could have level C or D depending on activity and loca-
tion. For example, level D would be appropriate for off-site, back-
ground activities. On-site activates during drilling and sampling
would require level C unless monitoring shows a downgrade is appro-
priate. At a minimum, on-site work would require use of PE Tyvek,
gloves, boots and hard hat at all times due to skin contact hazards
and active status of facility. This part of plan needs to be re-
vised to show information such as just described.
6. page C-4, Modification
Change Tyvek to PE Tyvek; change neoprene gloves to nitrile gloves;
describe splash protection; add hearing protection.
7. page C-5, Explosimeter/02 meter
Change action levels as follows:
1st bullet - "less than 20% LEL, watch carefully and be ready to shut
down as concentration approaches 20%".
2nd bullet - "50%" to "20%".
3rd bullet - "the UEL" to "100% of the LEL".
8. page C-5, HNu with 10.2 eV probe
Need an action level that would initiate work shut down based on or-
ganic vapor concentrations.
