The following was observed on the Facility Employee Training Certificates: -The Facility Employee Training Certificate dated 8/28/2019 states employe Elissett Tamayo was initially trained on 2/20/2019. -The"Initial Training Date"and"Date of Assuming Responsibility as a Facility Employee"sections were left blank on the Facility Employee Training Certificate dated 5/29/2019 and 5/6/2019. -The`Initial Training Date"section was left blank on the Facility Training Certificate dated 2/20/2019. All site employees must be trained prior to taking responsibility as a facility employee if after October 1, 2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee Training Certificate shall contain accurate information for all employees trained. Update the Facility Employee Training Certificate to contain the accurate information. This is a minor violation. 204 CCR 2712(i)Current monitoring plan approved by the EHD not found on site. An approved copy of the monitoring plans were not available for inspection. A copy of these plans shall be accessible on site at all times. Immediately locate and retain a copy on site. This is a minor violation. 205 CCR 2712(i), 2634(e)Failure to submit a current UST Response Plan and have it available onsite. An approved copy of the response plan was not available for inspection. A copy of this plan shall be accessible on site at all times. Immediately locate and retain a copy on site. This is a minor violation. 209 CCR 2716(a-e) Failure to conduct the designated UST operator visual inspection at least once every 30 days. 1)A designated operator inspection was performed on 11/12/2018 and the next inspection was performed on 12/13/2018, 31 days later. The designated operator visual inspections shall be conducted within 30 days or less of the previous inspection. Ensure that all designated operator inspections are performed within the required time frames. This is a minor violation. Overall Inspection Comments: Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, by 11/15/2019.
