Below is our response the 2 items noted in the enclosed inspection report, further action is required : ? 209- DO Report not submitted to site within 48 hours o DOs were trained to sign the owner operator line as they are CSI employees (not outside contractors) . We have since asked them to have the employee on site sign these . This will not occur again . ? 306 Jumper hoses on flex piping o This site will be closinq for construction 4- 15-2019 . Scope of work includes complete re- pipe of site to PVC will eliminate the flex piping and jumper hoses . We will need an extension for this issue . ? 319 91 Spill bucket does not hold 5 gallons ? This site will be closinq for construction 4. 15-2019 . Scope of work includes replacement of all spill buckets and fill pipes . We will need an extension for this issue. ? 320 87 and 91 products did not pass the overfill testing on 10- 13- 18 o These test results were received by your agency on 2/21 /19 1 will forward the Permit number as I am still waiting for a copy from the project manager .
