SAN 10 A Q U I N Environmental Health Department <br /> COUNTY RECEIVED FEB 21 <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> CHEVRON STATION #212470/2222 ` 3400 MACARTHUR DR , TRACY February 21 , 2019 <br /> Item * Remarks <br /> 209 CCR 2716(a-e) Failure to conduct the designated UST operator visual inspection at least once every 30 days. <br /> The designated operator failed to document the following : <br /> On the 10/30/18 designated operator ( DO) inspection report under <br /> -Section VI <br /> The OWNER / OPERATOR failed to provide a name, signature, and date within 48 hours of receiving the DO report <br /> under the ACKNOWLEDGMENT OF COMPLIANCE ISSUES section. The DO report was initially signed by the <br /> designated operator within the 48 hours however the operator did not sing the report until 2/8/ 19 . Ensure that the <br /> operator is signing the document within the require time frame provided by the regulations. <br /> During the monthly inspection, the designated operator shall review the alarm history for the previous month, check <br /> that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with <br /> documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing <br /> inspections at least once every 30 days at this facility and are including all of the required information on the reports. <br /> This is a minor violation . <br /> 306 CCR 2630(d ) Monitoring equipment is not installed or maintained to detect a leak at the earliest opportunity. <br /> The site has flexible primary and secondary piping for the 87 product, diesel product and 91 product Jumper hoses <br /> were observed in all product in all UDCs. Any potential release from the primary containment into the secondary <br /> containment in the 87 product, diesel product and 91 product piping does not flow into a UDC collection sump to <br /> detect a leak at the earliest opportunity. Immediately contact a properly licensed, trained, and certified contractor to <br /> ensure that any releases in the 87 product, diesel product and 91 product piping flows into the nearest sump for a <br /> leak to be detected at the earliest possible opportunity. Ensure that all monitoring equipment is maintained to detect <br /> a leak at the earliest opportunity. <br /> This is a Class II violation . <br /> 319 CCR 2635 , 2637 . 11 2665, 2712 No spill bucket, failed to contain 5 gallons, no means to keep it empty, or failed to be <br /> tested. <br /> The 91 Product spill container failed to have a minimum capacity of five gallons. All spill containers shall have a <br /> minimum capacity of five gallons. Immediately contact a properly licensed , trained , and certified contractor to repair <br /> or replace the spill container under permit and inspection of the EHD <br /> This is a minor violation . <br /> 320 CCR 2635, 2637 . 2, 2665 , 2712 Failure to comply with the installation , inspection , or record keeping of the overfill <br /> prevention . <br /> The 87 product, 91 product, and diesel product overfill prevention equipment was tested on 10/05/ 18 and results <br /> show fail result for the 87 product, 91 product and diesel product. The 87 product, 91 product, diesel product overfill <br /> prevention system is not currently functional . Immediately discontinue deposition of petroleum into this tank until the <br /> component is repaired or replaced by a properly licensed, trained, and certified contractor under permit and <br /> inspection of the EHD .
