S0 JOA U 1 Environmental Health Department <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> QUICKI KLEEN CAR WASH 707 E YOSEMITE AVE, MANTECA October 30, 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 209 CCR 2716(a-e) Failure to conduct the designated UST operator visual inspection at least once every 30 days. <br /> The following was observed on the designated operator's monthly inspection reports: <br /> -Section V of the reports were missing follow up actions about scheduling an Overfill Prevention Equipment <br /> Inspection. <br /> -Section VI of the report was missing date, name and signature of the owner/operator. <br /> -Secondary Containment test date was incorrectly stated. <br /> During the visual inspection,the designated operator shall check all required testing and maintenance for the UST <br /> system have been completed and shall check all required items on the report. Ensure that designed operators <br /> performing visual inspections at this facility are including all of the required information on the reports. <br /> This is a minor violation. <br /> 211 CCR 2715(c)(2) No employee present that were trained in the operation and maintenance of the UST system by a <br /> DO. <br /> Harry Singh has been working at this facility for more than a month. The designated operator employee training for <br /> Harry Singh was not performed. The designated operator shall train facility employees in the proper operation and <br /> maintenance of the UST system at least once every 12 months. The training shall be conducted before the <br /> individual performs the duties of a facility employees.The training shall include, but is not limited to: <br /> 1. Operation of the UST system in a manner consistent with the facility's best management practices <br /> 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br /> 4. Name of the contact person(s)for emergencies and monitoring equipment alarms <br /> Initial training shall also include a site-specific practical demonstration that provides both sharing information and <br /> evaluating knowledge for responding to emergencies, leak detection, monitoring alarms, spills and overfills. At least <br /> one of the facility employees present during operating hours shall have current training. Ensure that employees <br /> have been trained by the designated operator, maintain the list on site, and submit a copy of the training records to <br /> the EHD. <br /> This is a minor violation. <br /> 320 CCR 2635, 2637.2,2665, 2712 Failure to comply with the installation, inspection, or record keeping of the overfill <br /> prevention. <br /> Overfill prevention testing has not been conducted.This testing was required to be conducted by October 13, 2018. <br /> Immediately schedule this testing and provide 48 hours notification to the EHD.
