On the 3/7/19 designated operator(DO)inspection report under -Section I The DO failed to write the CERS ID and zip code -Section III The DO failed to add a description to all the components marked N/A from section VII through XI. -Section X The DO failed to mar YES, NO or N/A under the question"Are all sensors in containment sumps inspected located to detect a leak at the earliest opportunity?" -Section XI The DO failed to mark Yes or No to the question"Have all individuals performing facility employee duties received the required facility employee training within the past 12 months?" During the designated operator inspection,the designated operator shall review the alarm history for the previous month, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing inspections at least once every 30 days at this facility and are including all of the required information on the reports. This is a repeat violation, Class II. 313 HSC 25290.1, 25290.2, 25291 Failure to construct, operate, and maintain primary containment as product-tight. In dispenser 9/10 in the 87 product fuel meter a seepage was observed, indicating a leak in the primary piping. All primary containment for the UST system must be tight. Immediately have a properly licensed,trained, and certified contractor repair or replace the failed component under permit and inspection of the EHD. This is a repeat violation, Class II. 315 HSC 25291(e)Water in secondary containment not removed, analyzed, and properly disposed of(pre-Jul 2003). Liquid was observed in the 91 product fill sump. If water could enter into the secondary containment by precipitation or infiltration, it must be removed and disposed of properly. The service technician removed approximately 1-2 gallons of liquid from the 91 product fill sump. Ensure that all sumps and annular spaces are maintained free of liquid.
