UST Tank Information forms for the 87 product, the 91 product and the Diesel product are not current in CERS. The 91 product, and 87 product states secondary containment to be"fiberglass"for vapor and vent piping however single wall vent and vapor piping was observed at the time of inspection. Furthermore the diesel product states secondary containment to be"fiberglass"for vent piping however single wall vent piping was observed at the time of inspection. Any change of information must be updated in CERS within 30 days of the changes. Immediately log into CERS, update the required information, and submit for review by the EHD. This is a repeat violation, Class II. 202 CCR 2715(c)(3), 2715(c)(4) Failure to maintain the"Facility Employee Training Certificate"on-site(after October 1, 2018). "Facility Employee Training Certificate"attached to the 1/25/19, 5/15/19 and 6/11/19 was found on site missing the business name, address, city, zip code, name of the designated operator, signature from the DO and date of training.All site employees must be trained prior to taking responsibility as a facility employee if after October 1, 2018,or within 30 days of hire if prior to October 1,2018.The"Facility Employee Training Certificate"shall contain accurate information for all employees trained. Update the"Facility Employee Training Certificate"to contain the accurate information and submit a copy of the updated form to the EHD. This is a minor violation. 210 CCR 2716(f)DO monthly reports not maintained on site for 12 months or for 36 months(after October 1, 2018). The designated operator failed to document the following: On the 10/5/18,11/2/18, 11/28/18, 12/28/18, 1/25/19, 2/21/19, 3/21/19,4/16/19, 5/15/19, and 6/11/19 designated operator(DO) inspection report under -Section III The DO failed to add a description to all the components marked N/A or N from section VII through XI. On the 6/11/19 DO report, a work order is reference to but the work order was not attached to the DO report as requested under section VIII of the DO report. During the designated operator inspection,the designated operator shall review the alarm history for the previous month, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing inspections at least once every 30 days at this facility and are including all of the required information on the reports.
