The hazardous waste check box was not <br /> checked.The contingency plan must include: <br /> 1. Description of actions facility personnel will take in response to fires, explosions, or any sudden or <br /> non-sudden release of hazardous waste to air, soil or surface water at the facility; <br /> 2. Description of arrangements made with local police departments,fire departments, hospitals, contractors and <br /> State and local emergency response teams to coordinate emergency services; <br /> 3. List of names, addresses, and phone numbers(office/home/cell)of all persons qualified to act as emergency <br /> coordinator. Where more than one person is listed, one shall be named as the primary emergency coordinator and <br /> all others in order in which they will assume responsibilities; <br /> 4. List all emergency equipment,which includes location, physical description, and brief outline of its capabilities; <br /> 5. Evacuation routes for facility personnel where there is a possibility of evacuation; <br /> 6. Current telephone number for State Office of Emergency Services. <br /> If the facility has a Spill Prevention, Control and Countermeasures plan, or some other emergency or contingency <br /> plan,the owner or operator need only amend that plan to incorporate hazardous waste management provisions that <br /> are sufficient to comply with the requirements of this chapter. Immediately prepare or amend an existing plan to <br /> fulfill the requirements of this chapter. Submit a copy of the contingency plan to the EHD. <br /> This is a minor violation. <br /> 114 CCR 66262.40(a) Failed to keep signed copy of manifests from the designated facility for three years. <br /> Used oil was transported off site for disposal on 11/05/2018 without first preparing a uniform hazardous waste <br /> manifest. Forklift maintenance is being performed by Central Valley Lift Truck and taken off site by them. Service <br /> receipt is incomplete and is missing the amounts of waste, date waste was generated,where the waste was <br /> generated and where the waste is being transported to(street address). A generator shall prepare a Uniform <br /> Hazardous Waste Manifest on EPA Form 8700-22 prior to the transport of a hazardous waste for off-site transfer, <br /> treatment, storage or disposal. Ensure that a manifest is prepared each time hazardous waste is transported for off <br /> site transfer, treatment, storage or disposal or ensure that the service receipt contains complete information <br /> including the amounts of waste, date waste was generated,where the waste was generated and where the waste is <br /> being transported to(street address). <br /> Operator transported several hazardous wastes off site to 11855 White Rock Road in Rancho Cordova for disposal <br /> on 10/24/2018 on a CESQG receipt. The facility is not eligible to use a CESQG receipt because it generates more <br /> than 1 00k per month of hazardous waste. Facility disposed of approximately 1000 pounds per month of waste in <br /> 2017 and 220 pounds per month in 2018. Approximately 16,000 gallons of hazardous waste water is being treated <br /> monthly. A generator shall prepare a Uniform Hazardous Waste Manifest on EPA Form 8700-22 prior to the <br /> transport of a hazardous waste for off-site transfer,treatment, storage or disposal.
