They also provide personal data such as <br /> full name,date of birth,address at the time the license was issued or renewed, date of issue and the I <br /> date of expiration,physical description,type of license,previous Iicense number and restrictions on <br /> use. California will indicate whether another license was surrendered at the time of application. <br /> California records any infraction involving drugs or alcohol,as a permanent item in a driver's <br /> motor vehicle record. <br /> 1. Obtain Motor Vehicle Records(MVR) for each employee from the state in which the driver <br /> is licensed. <br /> i <br /> 2. Review the MVR to ensure that there have not been any accidents,violations or automobile s <br /> related convictions during the prior year that would cause a driver to no longer meet your <br /> company's Driver Eligibility Requirements. <br /> 3. Check the driver's license of all drivers to ensure that they are in possession of a license that is <br /> current,valid,and the correct classification of license for the type of the vehicle they are <br /> required to operate. <br /> 4. In reviewing each of the above per Imperial'Western Products,Inc. Company Driving € <br /> Policy will deme the minimum acceptable driving record to ensure that your vehicles are <br /> operated in the safest manner possible. <br /> ( Imperial Western Products,Inc.'s Company Driving Policy states that the employee call be <br /> restricted on driving privileges if there is all accrual of: <br /> > 2 points in 12 months,or <br /> > 3 points in 24 months, or <br /> > 4 points in 36 months,or <br /> > Any driving done on company business within your own or company owned vehicle whereas <br /> the employee was found to be under the influence of drugs or alcohol,or f <br /> > Any driving done on company business within your own or company owned vehicle whereas j <br /> the employee was found to be driving recldessly by a court of law,or <br /> > If there is any effect on the auto insurance rate of Imperial Western Products,Inc.. <br /> i <br /> > There will be no driving on company business within your own or company owned vehicle i <br /> whereas the employee has a restricted drivers license. <br /> Documentation for each of the above inquiries should be maintained in each of the driver's file. <br /> If you are entrusting your employee to transport merchandise or people,in your vehicle or theirs, a <br /> history of their performance behind the wheel is critical.Failure to obtain this information has <br /> been deemed to be negligent. 20% of the individuals with driving record violations are repeat <br /> offenders. In addition,the basic personal information can be extremely useful in cross-checking <br /> personal data provided by the applicant. <br />
