Time In: 1:35 pm <br /> Time Out: 2:05 pm <br /> Mobile Food Facility Complaint Inspection Report <br /> Name of Facility: EL SINALOENSE#6X71381 Date: 11/21/2017 <br /> Address: 1717 UNION ST, STOCKTON 95206 <br /> Owner/Operator: GERMAN COTA, JESUS GERMAN Telephone: (209)298-5416 <br /> Program Element: 1600- FOOD PROGRAM Complaint#: C00045141 <br /> DESCRIPTION OF COMPLAINT ALLEGATIONS <br /> The San Joaquin County Environmental Health Department received the following complaint: <br /> COMPLAINANT ALLEGES ON 11/17/2017 AT APPROXIMATELY 4:15 PM THEY NOTICED A MALE TEENAGER (15-16 <br /> YEARS OLD) THAT WAS WORKING ON THE FOOD TRUCK PREPARING FOOD GET OUT AND URINATE OPENLY <br /> BESIDE THE TACO TRUCK, DID NOT WASH HANDS AND BEGAN PREPARING FOOD AGAIN. THE TRUCK WAS <br /> PARKED OUTSIDE OF SUPER KING. <br /> VIOLATIONS AND CORRECTIVE ACTIONS <br /> Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br /> 113700.All violations must be corrected within specified timeframe. Violations that are classified as"MAJOR"pose an immediate threat to public health <br /> and have the potential to cause foodborne illness.All major violations must be corrected immediately. Noncompliance may warrant immediate closure of <br /> the food facility. <br /> OVERALL INSPECTION NOTES AND COMMENTS <br /> FOOD ITEM --LOCATION —TEMP°F--COMMENTS <br /> No Temperature Data Collected <br /> NOTES <br /> Spoke to owner who was not aware of complaint. He stated that he has commissary letter from Super King <br /> Observation: Hand sink has hoUcold water, soap and paper towels. Rest room letter was from Pizza Restaurant which is now <br /> closed. <br /> Correction: Owner went to Super King and had letter signed at time of visit.
