To determine whether the operation of your facility complies <br /> with Section 18420 (a) (4) , (a) (5) , (b) or (c) , the following <br /> questions must be answered in addition to the remainder of this <br /> form: <br /> a. For those operators intending to comply with 18420 (a) (4) : <br /> ❑ For waste tires stored in an enclosed structure, <br /> does the building and the method of storing the waste tires <br /> conform to "The Standard for Storage of Rubber Tires" , National <br /> Fire Protection Association, NFPA 231D-1989 edition, published by <br /> the National Fire Protection Association NFPA 231D, as specified <br /> in Section 17356 (a) , Article 5 .5, Chapter 3 : <br /> YES NO <br /> If the answer to the above question is "No" , attach the <br /> local fire authority approval along with their requirements. <br /> If the answer is "No" and the local fire authority will not grant <br /> an approval, then the facility shall be closed in accordance with <br /> Article 6 of this Chapter. <br /> b. For those operators intending to comply with 18420 (a) (5) : <br /> ❑ For waste tires stored in movable containers, <br /> describe the type of containers to be used and how the operator <br /> will comply with section 17356 (b) , Article 5 . 5 of Chapter 3 . <br /> CIWMB 500 (10/92) <br />
