This is a Class II violation. 104 CCR 2711 UST operating permit application for facility and tank information not submitted or current. UST Facility Operating Permit is not current in CERS. Any change of information must be updated in CERS within 30 days of the changes. Immediately log into CERS, update the required information, and submit for review by the EHD. UST Tank Information forms for the [note which ones] are not current in CERS. The tank forms for all three tanks indicate the vent piping is single-walled, but state there is a vent sump on site. This is a Class II violation. 105 CCR 2632(d)(1), 2634(d) Failed to have an approved UST monitoring plan. The monitoring plan is not current and/or not approved by the EHD. In the UDC Monitoring section of the Monitoring Plans for all three tanks, Detection of Leak, UDC Leak Alarm, and Failure/Disconnection options were all left blank. In the UDC Monitoring section of the Monitoring Plans for all three tanks, Panel Manufacturer and Model have been filled in with Bravo float information. The dispenser leak monitoring system is not connected to a monitoring panel. The monitoring plan must be uploaded to the California Environmental Reporting System (CERS). Immediately log into CERS, make the necessary changes, and submit for review by the EHD. This is a Class II violation. 106 CCR 2632(d)(2), 2634(e) Failed to have an approved UST response plan. The response plan is not current and/or not approved by the EHD. The response plan must be uploaded to the California Environmental Reporting System (CERS). Immediately log into CERS, upload the completed form, and submit for review by the EHD. Ensure a copy is available for inspection at any time. This is a Class II violation. 107 CCR 2632(d)(1)(C) Plot plan/site map not submitted or failed to completely show where monitoring is performed. An accurate UST Monitoring Site Plan was not submitted. The Monitoring Site Plan (map) does not include annular sensors. A site plan must be submitted identifying the locations where monitoring will be performed.
