The 89 and 91 leak <br /> detectors could not be tested due to lack of fuel. These tests are required once every 12 months. Immediately <br /> schedule these tests and provide 48 hours notification to the EHD. Please be aware that a follow up inspection will <br /> be charged at our current hourly rate. <br /> This is a Class II violation. <br /> 115 CCR 2715(a)(1)(B), 2715(a)(3)"Designated Underground Storage Tank Operator Identification Form" not submitted <br /> within 30 days. <br /> A new designated operator was hired and notification was not provided to the EHD within 30 days of the change. <br /> Terek Naward began conducting designated operator inspections on 7/28/18. Any changes shall be reported to the <br /> EHD within 30 days. Immediately log into the California Environmental Reporting System (CERS) at <br /> http://cers.calepa.ca.gov and upload the notification identifying all the designated operators for this facility. <br /> This is a Class II violation. <br /> 118 CCR 2715(a)(1)(A), 2715(a)(2) Failure to submit the "Underground Storage Tank Statement of Understanding and <br /> Compliance Form". <br /> The Underground Storage Tank Statement of Understanding and Compliance Form for the new owner was not <br /> found in CERS. This form must be uploaded to the California Environmental Reporting System (CERS). <br /> Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov and upload <br /> the Underground Storage Tank Statement of Understanding and Compliance Form for this facility. <br /> This is a Class II violation. <br /> 201 CCR 2712(b) Failure to maintain monitoring, maintenance, and follow up action records. <br /> 2018 monitoring system certification test results were not found on site. These records shall be maintained on site <br /> for at least three years. Monitoring records include: (1) date and time of all monitoring or sampling; (2) monitoring <br /> equipment calibration and maintenance records; (3) results of any visual observations; (4) results of sample analysis <br /> performed a lab or in the field; (5) logs of all readings of gauges or other monitoring equipment, ground water <br /> elevations, or other test results; (6) results of inventory readings and reconciliations. Immediately locate and <br /> maintain all missing maintenance and monitoring records for the last three years on site. <br /> This is a Class II violation. <br /> 204 CCR 2712(1) Current monitoring plan approved by the EHD not found on site. <br /> An approved copy of the monitoring plans were not available for inspection. A copy of these plans shall be <br /> accessible on site at all times.
