The designated operator shall train facility employees for <br /> which he or she is responsible in the proper operation and maintenance of the UST system once every 12 months. <br /> The training shall include, but is not limited to: <br /> 1. Operation of the UST system in a manner consistent with the facility's best management practices <br /> 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br /> 4. Name of the contact person(s)for emergencies and monitoring equipment alarms <br /> Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of <br /> the training records to the EHD. <br /> This is a Class II violation. <br /> 309 CCR 2636(f)(1) Monitoring in sump/ UDC failed to activate an audible and visual alarm or stop the flow product. <br /> The Bravo floats and chains in UDCs 1/2 north, 3/4 north, 5/6 north, and 7/8 north were all tripped prior to arrival <br /> and were not capable of detecting fuel leaks. The chain in UDC 3/4 south was disconnected and unable to detect <br /> fuel leaks. All monitoring equipment shall be maintained to activate an audible and visual alarm or stop the flow of <br /> product at the dispenser when it detects a leak. Correct immediately by having a properly licensed, trained, and <br /> certified contractor repair or replace the failed component with a functional component(LG 113-x listed, if <br /> applicable) and obtain a permit within one business day from the EHD, if necessary. If the failed component can't <br /> be replaced immediately, there is a possibility that the UST system may be red tagged to prevent fuel inputs. <br /> This is a Class II violation. <br /> 310 CCR 2636(f)(4) Piping is not fail-safe outside UDCs or fails to stop flow of product at dispenser inside UDCs. <br /> The positive shutdown on the 89 and 91 STP sump sensors could not be tested. These sensors do not currently <br /> meet the requirements for annual line tightness testing exemption. Immediately provide the necessary conditions <br /> for these sensors to meet the exemption, or schedule an annual line tightness test and provide 48 hours notification <br /> to the EHD. <br /> This is a Class II violation. <br /> 320 CCR 2635, 2637.2, 2665, 2712 Failure to comply with the installation, inspection, or record keeping of the overfill <br /> prevention. <br /> Overfill prevention testing has not been conducted. This testing was required to be conducted by October 13, 2018. <br /> Immediately schedule this testing and provide 48 hours notification to the EHD.
