An approved copy of the response plan was not available for inspection. A copy of these plans shall be accessible on site at all times. Immediately locate and retain a copy on site. This is a Class II violation. 209 CCR 2716(a-e) Failure to conduct the designated UST operator visual inspection at least once every 30 days. A designated operator inspection was performed on October 28, 2018 and the next inspection was performed on November 28, 2018, 31 days later. The designated operator visual inspections shall be conducted within 30 days or less of the previous inspection. Ensure that all designated operator inspections are performed within the required time frames. The designated operator failed to check and note all required items on the visual inspection reports. The 10/28/18, 11/28/18, 12/28/18, and 1/27/19 inspection reports had the following issues: -Line and tank tightness testing was left blank. -Secondary containment testing was left blank with a "?" in the date section. -No overfill prevention test date was included. -No CERS ID was listed. -Section V was left blank. -The reports were unsigned by the owner/operator. During the visual inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and shall check all required items on the report. Ensure that designated operators performing visual inspections at this facility are including all of the required information on the reports. This is a Class II violation. 210 CCR 2716(f) DO monthly reports not maintained on site for 12 months or for 36 months (after October 1, 2018). The March 2018,April 2018, and June 2018 designated operator monthly inspection reports were not found on site. Designated operator monthly inspection reports for the previous twelve months shall be retained on site. Locate and ensure that copies of the previous twelve months of designated operator monthly inspection reports are maintained on site. Submit copies to the EHD. This is a Class II violation.
