A complete report was <br /> amended and finished, and replaces the initial checklist on 10/29/2020. <br /> -Violation 108 was removed from the inspection report, since it was inadvertently marked on the inspection <br /> checklist. <br /> -According to Winona Blass, Supervisor, all employees receive hazardous waste management training in order <br /> to clean up spills of hazardous chemicals and be able to place damaged hazardous items into the hazardous <br /> waste storage containers. <br /> -Three blue 5-gallon containers, two of which were empty and one of which contained a white cloth, piece of <br /> plastic, and a zip tie, were observed next to the hazardous waste containers. The 5-gallon containers were <br /> labeled as Salvage Ground Ship Only and with a FedEx shipping label stating FedEx Ground Package Returns <br /> Program and the shipping address, BBW Store Returns, 7 Limited Parkway Dock Door 80, Attn: IHRL BBW <br /> RET, Reynoldsburg, OH 43068. According to Jessica Graham, Store Manager, these 5-gallon containers were <br /> used to contain and ship the facility's damaged hazardous items via FedEx, before the facility began using the <br /> black poly drums, which are now used to store the facility's damaged hazardous items as hazardous waste. <br /> Jessica did not know where the 5-gallon containers of damaged hazardous items/hazardous waste were <br /> shipped to. <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by 11/28/2020. <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br /> of having them sign. Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate <br /> ($152).
