Please provide the following additional information and address the following comments: <br /> 1. The Services Request needs to be signed by the billing party. The Service Request indicates that 7-Eleven will be <br /> the responsible billing party but it was signed by Robert Forline of PM Design Group, Inc. Please provide a <br /> revised Service Request form signed by the billing party. <br /> 2. Page 3 of the Application for Underground Storage Tank Installation Permit: contractor information and Fire <br /> Permit information were not provided. <br /> 3. Page G1.0 incorrectly states that the pipe with the longest run is diesel. This is inconsistent with the plan <br /> drawings. <br /> a. Your response:This page in the plan drawings does not show the length of any product piping run. The <br /> longest run is indicated on sheet GOA on the tank burial chart. Diesel is the longest run, at <br /> approximately 162'. The second longest run is the premium unleaded product piping, at approximately <br /> 158'. <br /> b. Comment: Indicate which product pipe has the longest total pipe length and provide the pipe length. <br /> 4. In the engineered plans (page G5.0),the Veeder Root tubing doesn't appear to be properly connected and it has <br /> a ball valve <br /> a. Your response:The third available spot on a 3-way ball valve provides an additional test port within the <br /> vacuum system for testing purposes without having to remove the vacuum tubing <br /> b. Comment: No additional shut off valves should be installed between the monitored interstitial spaces <br /> and their respective sensors. Remove all unnecessary ball valves from the plans. <br /> S. The plans indicate ball valves will be placed on vacuum tubing at multiple locations. No additional shut off <br /> valves should be installed between the monitored interstitial spaces and their respective sensors. Remove all <br /> unnecessary ball valves from the plans. <br /> a. The test ports <br /> output at the ball valve will be closed until any necessary testing on the vacuum system is required. <br /> b.
