® . a 2111 il 0 . • i <br /> 4 . 0 MEDICAL SURVEILLANCE REQUIREMENTS <br /> This contractor has a program in place that requires medical surveillance of <br /> employees that work in hazardous waste clean - up operations covered under Title 8 <br /> Section 5192 (a ) ( 1 ) (A) through ( E ) and whose work may include : <br /> • Exposure or potential exposure to hazardous substances or health hazards at or <br /> above the Permissible Exposure Limit ( PEL) , or if there is no PEL , above the <br /> published exposure levels for these substances without regard to the use of <br /> respirators for 30 days or more a year ; <br /> • Wearing a respirator during any part of a day for a period of 30 days or more in a <br /> year ; and <br /> • Performing duties of a HAZMAT team . <br /> Employees shall have medical surveillance : <br /> • Prior to assignment or employment ; <br /> • At least once every twelve months for each employee covered , unless the <br /> attending physician believes a longer interval is appropriate , not greater than <br /> biennially . <br /> • At termination of employment or reassignment to an area where the employee <br /> would not be covered if the employee has not had an examination within the last <br /> six months ; and <br /> • If any employee that is injured , becomes ill or develops signs or symptoms due to <br /> possible overexposure involving hazardous substances or health hazards from an <br /> emergency response or hazardous waste operation , will be examined by a <br /> physician as soon as possible upon notification by the employee . <br />
