S0 JOA U 1 Environmental Health Department
U ; i y I AMENDED
11/2/2020
Small Quantity Hazardous Waste Generator Inspection Report
Facility Name: Facility Address: Date:
BATH & BODY WORKS#65 5308 PACIFIC AVE, STOCKTON October 28, 2020
SUMMARY OF VIOLATIONS
(CLASS I,CLASS II,or MINOR-Notice to Comply)
Item# Remarks
605 CCR 66262.34(f) Failed to completely label containers or portable tanks of hazardous waste.
OBSERVATION: Hazardous waste containers were observed on-site with incomplete labeling. The containers
were:
1.A 30-or 55-gallon black poly drum, storing damaged liquid hand and body soaps, anti-bacterial hand sanitizer
gels, scented candles, and fragrance mists, was missing the words"Hazardous Waste", name and address of
generator, hazardous properties, physical state, composition (contents), and accumulation start date. Gloria Alonzo,
Store Manager, labeled the drum with the required information at the time of inspection.
2.A tote storing damaged scented candles, hand soaps, fragrance mists, and fragrance wall plug-ins,was missing
the words"Hazardous Waste", name and address of generator, physical state, and accumulation start date.
3.A tote storing a damaged fragrance mist was missing the words"Hazardous Waste", name and address of
generator, physical state, and accumulation start date.
REGULATION GUIDANCE: All hazardous waste containers shall be marked with the following information: the
words"Hazardous Waste", name and address of generator, hazardous properties, physical state, composition
(contents), and accumulation start date.
CORRECTIVE ACTION: Immediately label the containers as required for hazardous waste. Provide a corrective
action statement and supporting documentation to the San Joaquin County Environmental Health Department
(EHD)within 30 days.
This is a Class II violation.
Overall Inspection Comments:
Notes:
-An inspection checklist was provided to the facility operator on the day of inspection. A complete report was
amended and finished, and replaces the initial checklist on 11/2/2020.
-According to Gloria Alonzo, Store Manager, all employees receive hazardous waste management training in
order to clean up spills of hazardous chemicals and be able to place damaged hazardous items into the
hazardous waste storage containers.
-At least ten blue 5-gallon containers and three white 5-gallon containers, were observed on top of the facility's
electrical box. According to Gloria Alonzo, Store Manager, these 5-gallon containers are empty and were used
to contain and ship the facility's damaged hazardous items before the facility began using the black poly drums
and totes, which are now used to store the facility's damaged hazardous items as hazardous waste. Gloria did
not know where the 5-gallon containers of damaged hazardous items/hazardous waste were shipped to. The
5-gallon containers appeared to be labeled with a Fedex shipping label.
Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement
documenting the corrective actions that have been or will be taken for each violation, and any supporting
paperwork, by 12/2/2020.
To minimize person to person contact EHD is choosing to write the name of person receiving the report instead
FA0024348 PR0542379 SCO01 10/28/2020
EHD 22-01 Rev.9/16/2020 Page 9 of 10 Small Quantity Hazardous Waste Generator OIR
1868 E.
