The gas concentration of methane <br /> observed from the bar hole punch was written as an area of concern for section 20919 <br /> of Title 27 California Code of Regulations(27 CCR), Gas Control. The inspection <br /> report documented that a work plan must be provided to this LEA for installation of <br /> gas migration probes within 90 days by November,2003. (Exhibit A) <br /> IV. On August 25th, 2003,Jay Kamine of URS spoke with this LEA to inform that URS <br /> will be contracted to conduct any work required by this LEA. This LEA requested to <br /> URS that permanent monitoring wells for gas migration are to be installed on each <br /> property boundary. <br /> V. On September 18th,2003,this LEA sent a written notice requiring Tandy <br /> Corporation to install four permanent monitoring wells for gas migration at each <br /> property boundary of the site. (Exhibit B) <br /> VI. On November 17th, 2003,this LEA received a permit application faxed from URS <br /> for the construction and installation of monitoring wells for gas migration at the site. <br /> VII. On November 18th,2003,this LEA received a Work-Plan from URS for the <br /> installation of monitoring wells for gas migration at the site. <br /> VIII. On December 11th, 2003,this LEA received a Draft Addendum to the Work-Plan <br /> from URS for the installation of monitoring wells for gas migration at the site. <br /> IX. On December 12th, 2003,this LEA issued a permit to the Tandy Corporation for the <br /> construction of the four monitoring wells for gas migration at the site. (Exhibit Q. <br /> X. On December 15th,2003,this LEA received an Addendum to the Work-Plan from <br /> URS for the installation of the monitoring wells for gas migration at the site. <br /> XI. On April 2nd, 2004,this LEA sent a faxed copy of the permit to Jeff Brooks of URS <br /> for the construction and installation of the monitoring wells for gas migration. <br /> XII. On April 7th, 2004,this LEA inspected the construction and installation performed <br /> by URS of the four monitoring well for gas migration at the site. <br /> XIII. On April 8th, 2004,this LEA advised Jeff Brooks of URS to add appropriate valves <br /> to the four monitoring wells to perform gas monitoring. <br /> XIV. On April 16th,this LEA observed that the four monitoring wells had new locks and <br /> were properly secured. <br /> XV. On May 6th, 2004,this LEA received an email message from Jay Kamine of URS <br /> stating to Jeff Brooks about the provision of keys for access of the four monitoring <br /> wells. The message stated that the site owner is willing to provide unlimited access <br /> for the LEA to inspect and conduct gas monitoring at the site,but would like their <br /> Notice and Order 04-01 Page 2 of 7 World Enterprises Site <br /> October 25,2004 <br />
