Below is the information that you requested for Ulta Beauty stores#579 (Tracy) and #1143 <br /> (Manteca). <br /> Ulta#579: <br /> 1. Violation 102:Submit a statement and supporting documentation explaining how you have determined the <br /> Barbicide and Ship Shape solutions to be hazardous or non-hazardous waste. <br /> Response: Ulta Beauty manages Barbicide and Ship Shape containers as hazardous waste. These items are placed into <br /> the non-RCRA"State Managed Waste" hazardous waste bins. Liquid Barbicide and Ship Shape solutions are not <br /> considered hazardous waste. These liquid solutions are similar to shampoo and conditioner used on clients. Please note <br /> that salons, barbershops and beauty schools in San Joaquin County and throughout California, do not manage these <br /> liquid solutions as hazardous waste. However, unlike most (if not all) salons, barber shops and beauty schools in San <br /> Joaquin County and throughout California, Ulta Beauty has a hazardous waste program at its facilities and manages <br /> Barbicide and Ship Shape containers as State-Managed hazardous waste. <br /> 2. Violation 106: <br /> 1. The topics covered on the Hazardous Waste Program Certificate of Employee Training does not include <br /> how employees are trained to clean up a hazardous waste spill and how to manage the waste generated <br /> from the clean-up. Submit the training material that covers hazardous waste spill clean-up and <br /> management, and proof of employee training on this topic. <br /> Response: Attached is the Hazardous Waste Program Process, Containers, Labels&Spills policy that Ulta Beauty store <br /> managers review and are trained on which includes the process for the cleanup of spills. See pages 11-13. Note that <br /> only store managers are allowed to clean up spills and only managers perform tasks associated with the hazardous <br /> waste program as part of their job responsibilities. Also attached is a signed certificate of training for store managers at <br /> #579 which confirms training on cleaning up hazardous waste spills. <br /> 2. The certificate of employee training sign-off sheet shows that most of the employees were trained prior <br /> to the date of inspection. Submit proof of employee re-training dated after the inspection. <br /> Response: See the attached certificate of employee re-training for store managers at#579 dated after the inspection. <br /> 3. Jasmin Sekhon, Merchandise Service Coordinator,was not trained on the hazardous waste program. <br /> Submit proof of Jasmin's hazardous waste program training. <br /> Response: See the attached certificate of employee re-training for store managers at#579.
