3. Violations 403 and 605: Submit a statement explaining current storage and labeling for hazardous wastes found
behind the front cash registers.
Response: Hazardous waste is not maintained behind the front cash registers at Ulta Beauty stores. Hazardous waste is,
instead, located in the store's backroom. Note that when a guest returns a product, it is temporally placed behind the
cash registers. No waste or hazardous waste determination has been made on these items. At the end of each day, a
store manager takes these items to the store's backroom and audits these items. Some of these items, such as returned
products that have never been opened, can still be salable and are not a waste, and therefore, not a hazardous waste. If
the manager determines that an item is no longer salable, it then becomes a waste. If the waste is a "hazardous waste"
then it is immediately managed as a hazardous waste and placed into the appropriate hazardous waste container.
4. Submit a statement and supporting documentation explaining which company/companies have been hired to
perform store item spill clean-ups, such as nail polish spills, and how these companies managed the waste
generated from these clean-ups.
Response: Store managers are trained to perform spill cleanups. See above and the attached certificate of re-training
for store managers at#579.
Ulta#1143:
1. Violation 102:Submit a statement and supporting documentation explaining how you have determined the
Barbicide and Ship Shape solutions to be hazardous or non-hazardous waste.
Response: See the response to Violation 102 above for Store#579.
2. Violation 106:Submit proof of employee re-training by the General Manager, dated after the date of inspection.
Response: See the attached certificate of employee re-training for store managers at#1143, including the General
Manager dated after the inspection.
Please let me know if you have any questions. I am happy to discuss any of these responses in a telephone call if you
need any clarification. I hope you, your family and the entire San Joaquin CUPA team have a safe and happy holiday
season.
Best regards,
Jeremy
From: Florido, Elianna [EHD] <eflorido@sjgov.org>
Sent: Friday, December 18, 2020 12:38 PM
To: Hojnicki,Jeremy<jhojnicki@ulta.com>
Cc:Anzar,Tracey<tanzar@ulta.com>
Subject: [EXTERNAL] Ulta stores#579 and #1143 - 2019 Hazardous Waste Inspection Follow Up
Importance: High
CAUTION:This email originated outside of the Ulta organization.
