SAN J O A R N Environmental Health Department <br /> i , �V <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> A&A GAS FOOD MART 1330 E YOSEMITE AVE, MANTECA November 03, 2021 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 208 CCR 2716(a-e) Failure to comply with the designated operator inspection requirements. <br /> 1)OBSERVATION:The designated operator failed to check and note the correct test dates for the overfill <br /> prevention and secondary containment testing on the designated operator inspection reports. <br /> -Routine overfill prevention testing was conducted on 12/28/2018. <br /> -Routine secondary containment testing was conducted on 1/12/2021. <br /> REGULATION GUIDANCE: During the inspection, the designated operator shall check that all required testing and <br /> maintenance for the UST system have been completed and document the dates they were done. <br /> CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of <br /> the required information on the reports. <br /> --------------------------------------------------------- <br /> 2)OBSERVATION:The designated operator failed to document all the alarms from the attached alarm history on <br /> the 2/11/2021 designated operator inspection report and failed to check that they were responded to appropriately. <br /> The missing alarms include: L7(UDC 1/2)sensor out alarm <br /> REGULATION GUIDANCE: During the inspection,the designated operator shall review the alarm history since the <br /> previous inspection, check that each alarm was documented and responded to appropriately, and attach a copy of <br /> the alarm history with documentation taken in response to any alarms to the inspection report. <br /> CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of <br /> the required information on the reports. <br /> --------------------------------------------------------- <br /> 3)OBSERVATION:The designated operator failed to check and note all required items on the visual inspection <br /> reports.The incorrect and missing information included the following from the inspection reports: <br /> -Section 5 was missing owner/operator's response about documenting alarms in the alarm log. <br /> -The 7/1/2021 designated operator inspection report had the wrong monthly inspection date.
