SAN J O A R N Environmental Health Department <br /> i , �V <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> A&A GAS FOOD MART 1330 E YOSEMITE AVE, MANTECA November 03, 2021 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> --------------------------------------------------------------------------- <br /> 2)OBSERVATION:The mechanical overfill prevention device in the diesel tank failed during during the 12/28/2018 <br /> testing and was not replaced until 1/8/2021. <br /> REGULATION GUIDANCE:All UST systems must be equipped with an approved,functional overfill prevention <br /> device at all times during delivery operations. <br /> CORRECTIVE ACTION: Ensure all testing is conducted within the required time frames. <br /> --------------------------------------------------------------------------- <br /> 3)OBSERVATION:The routine overfill prevention testing for all three tanks was due in October 2021 but test has <br /> not been conducted. <br /> REGULATION GUIDANCE: Overfill prevention equipment inspections were required to be conducted by October <br /> 13, 2018, and every 36 months thereafter, and within 30 days of completion of a repair. <br /> CORRECTIVE ACTION: Immediately schedule this testing and provide 48 hours notification to the EHD. Ensure all <br /> future testing is conducted within required time frames. <br /> This is a Class I violation. <br /> Overall Inspection Comments: <br /> The monitoring system certification was conducted on 11/3/2021. An inspection checklist was provided to the <br /> facility operator on the day of inspection. A complete report was finished on 11/8/2021 and replaces the initial <br /> checklist. <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by 12/8/2021. <br /> NOTE: The monitoring plans for all three tanks incorrectly list the monitoring panel for the UDC monitoring as a <br /> TLS 350. The panel on site is a TLS 350 Plus <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br /> of having them sign. Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate <br /> ($152).
