The facility's tank information <br /> form for all three tanks state that site is using ball floats as the only means of overfill protection. The facility has <br /> single walled vent piping,therefore, can not use ball floats as the only means of overfill protection. <br /> -On 12/28/2018, all three tanks failed the overfill prevention equipment inspection.The mechanical overfill <br /> prevention device in the diesel tank was set at 99%capacity and not at 95%or below as required.The 87 and 91 <br /> tanks did not have a mechanical means of overfill nor was there an audible/visual alarm to alert the fuel delivery <br /> truck driver when the tanks have reached above the 90%capacity. <br /> -On 11/12/2019, a repair permit(SR0080109)was approved to replace the 91 and diesel tank drop tubes but no <br /> work was conducted. <br /> -On 9/30/2020, a repair permit(SR0082633)was approved to replace the 87, 91 and diesel drop tubes and shut-off <br /> valves(flappers). During the 12/28/2021 service request inspection, the service technician onsite verified that the <br /> diesel tank did not have ball float installed but had an OPW drop tube with a shut-off valve.The 87 and 91 tanks had <br /> ball floats and straight drop tubes. Per technician,the 87 and 91 tanks passed and the flapper in the diesel tank <br /> failed. The service technician only took measurements of the tanks and did not perform the drop tube <br /> replacements. No tank charts were onsite for verification,when asked about tank charts the service technician <br /> stated that they were at the"shop."The repair permit was not conducted and Ghulam Ali (Operator)was notified <br /> indicating that the facility was not approved to receive fuel in the 87, 91, and diesel tanks. <br /> -On 1/8/2021, the diesel drop tube was replaced under repair permit number SR0082633.An OPW drop tube was <br /> installed in the diesel tank instead of the EMCO drop tube the permit was approved for. (An amended scope of work <br /> has not been submitted).The repair permit was not finalized at that time,flappers were not installed in the 87 and <br /> 91 tanks.The facility was not approved to receive fuel in the 87 and 91 tanks.A service request inspection was <br /> emailed to the Ghulam Ali (Operator)indicating the that the facility was not approved to receive fuel in the 87 and 91 <br /> tanks prior to EHD's approval due to the the replacement of drop tubes not being performed.As of today, <br /> 11/8/2021,the 87 and 91 tanks do not have an overfill prevention system <br /> REGULATION GUIDANCE:All underground storage tanks shall be equipped with an overfill prevention system that <br /> does not allow for manual override and meets one of the following requirements: <br /> 1. Alert the transfer operator when the tank is 90 percent full by restricting the flow into the tank or triggering an <br /> audible and visual alarm; or <br /> 2. Restrict delivery of flow to the tank at least 30 minutes before the tank overfills, provided the restriction occurs <br /> when the tank is filled to no more than 95 percent of capacity; and activate an audible alarm at least five minutes <br /> before the tank overfills; or <br /> 3. Provide positive shut-off of flow to the tank when the tank is filled to no more than 95 percent of capacity;or, <br /> 4. Provide positive shut-off of flow to the tank so that none of the fittings located on the top of the tank are <br /> exposed to product due to overfilling. <br /> CORRECTIVE ACTION: Immediately contact a properly licensed,trained, and certified contractor to install an <br /> approved overfill prevention system. Replacement of a mechanical flapper valve requires a permit and inspection <br /> by the EHD. If an external audio/visual alarm is replaced, provide a copy of the service receipt to the EHD.
