Hazelton Avenue <br /> 6 Stockton , California 95205 <br /> 7 <br /> A photocopy of the check shall be sent to the following CUPA personnel assigned to this <br /> 8 <br /> matter at the above - identified address : Michelle Henry , San Joaquin County Environmental <br /> 9 <br /> Health Department . If Respondent fails to make payments as provided above , Respondent <br /> 10 <br /> agrees to pay interest at the rate established pursuant to HSC § 25360 . 1 and to pay all <br /> 11 <br /> costs incurred by the CUPA in pursuing collection including attorney ' s fees . <br /> 12 <br /> 13 OTHER PROVISIONS <br /> 14 12 . 1 . Additional Enforcement Actions : By agreeing to this Consent Order , <br /> 15 the CUPA does not waive the right to take further enforcement actions , <br /> 16 except to the extent provided in this Consent Order . <br /> 17 12 . 2 . Penalties for Noncompliance : Failure to comply with the terms of this <br /> 18 Consent Order may subject Respondent to civil penalties and /or punitive <br /> 19 damages for any costs incurred by the CUPA or other government agencies <br /> 20 as a result of such failure , as provided by HSC § 25404 . 1 . 1 and other <br /> 21 applicable provisions of law . <br /> 22 <br /> 12 . 3 .
