Please correct the <br /> following deficiencies and submit immediately. <br /> Violation #5: Hazardous Materials Inventory is incomplete. <br /> • Approximately 552 gallons of Peroxide Multi-Surface Cleaner& Disinfectant was at the facility at the time of the inspection and <br /> only 144 gallons reported. Entered the maximum quantity of the chemical that the facility may have at any time. <br /> • 110 gallons of Waste Refrigeration Oil is not listed <br /> • 600 gallons of Diesel Exhaust Fluid (DEF) is not listed: the chemical form shows that the fuel island is the only location for this <br /> chemical. Please correct. <br /> • 1900 gallons of Diesel for the Emergency Diesel Generator is not listed <br /> • Some of the liquid chemicals are reported in pounds.All the liquid chemicals shall be reported in gallons except for extremely <br /> hazardous substances(EHS); the only EHS at this facility is the Anhydrous Ammonia.There are still a list of liquid chemicals <br /> reported in gallons such as Liquid Margarine, Sanitizers, Safeguard Hand Soap, DEODORIZER/REST SPRAY,etc.. <br /> Note:You reported chemicals to the inventory in quantities that are less than the threshold reportable quantities(less than 55 gallons of <br /> liquid and 500 pounds of Solid).The facility is billed $17 per chemical as part of the annual permit. It is recommended to discard those <br /> chemicals. If you believe these chemicals may exceed the threshold reportable quantities at any time, enter the maximum amount of <br /> the chemical. <br /> Violation#6: The updated site map is incomplete.The new site map does not address the locations of the following <br /> elements: <br /> • Chemical locations(Aisle 30,Aisle 31, Lead Acid Batteries Area,Anhydrous Ammonia, Oil and Waste Oil, Emergency Diesel <br /> Generator, and Water Treatment Chemicals) <br /> • Evacuation Staging Area(s) <br /> • Emergency Shut offs(Ammonia shut offs,fuels shut offs, etc...) <br /> • Emergency Response Equipment( Fire Extinguishers, Eye Wash/Shower Station, etc..) <br /> • Emergency Gate: the site maps submitted previously shows that the facility has an emergency gate.Was this an error? <br /> • Internal Road <br /> Note: If a site map element is not applicable for your facility then list it on the map and label as"NA". For Example, internal road: NA or <br /> facility does not have internal road(s). <br /> Violation #11: The received training records are not current and incomplete. <br /> All employees shall be trained in safety procedures in the event of a release or threatened release of a hazardous material, <br /> including, but not limited to,familiarity with the following plans and procedures: <br /> (A) Immediate notification contacts to the appropriate local emergency response personnel and to the unified <br /> program agency. <br /> (B) Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property,or the environment. <br /> (C) Evacuation plans and procedures, including immediate notice,for the business site. <br /> This training shall be documented electronically or by hard copy and shall be made available for a minimum of three <br /> years. <br /> For examples your last HAZCOM training was completed in 2018.Adam Baltzer, John Sheridan, and Pedro Aguilar do not have the <br /> HAZCOM Training.
