The following is an itemized list of hazardous waste violations that have not been <br /> addressed for QUEST INDUSTRIES LLC as of January 11, 2021. <br /> Open violations from November 04, 2020 inspection <br /> (4)employee-signed or-certified records that document that the training required under subsections(a), (b), and(c) <br /> of this section has been given to, and completed by, each employee. <br /> (e)The owner or operator shall maintain training records on current personnel until closure of the facility and training <br /> records on former employees for at least three years from the date the employee last worked at the facility. <br /> Personnel training records may accompany personnel transferred within the same company. <br /> CORRECTIVE ACTION: Immediately provide the complete training documentation to the San Joaquin County <br /> Environmental Health Department(EHD)within 30 days. The documentation must include a list of training topics, <br /> date of training,job title, name of employee,written job description(qualifications and duties), and employee signed <br /> (or certified)record that documents the training. <br /> ❑ This violation was corrected ❑This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#124-Failed to report program data in the California Environmental Reporting System(CERS). <br /> OBSERVATION: This business's hazardous waste activities have not been reported on the California <br /> Environmental Reporting System(CERS)website. The facility marked that they treat hazardous waste on site. No <br /> submittal have been made with the treatment details. <br /> REGULATION GUIDANCE: Each regulated business shall report program data electronically on the CERS <br /> website. <br /> CORRECTIVE ACTION: Immediately report the treatment activities on the CERS website.
