MFS V.P Tony Grizelj had been in contact with <br />Lydia Baker — SJC EHS prior to the July 20, 2021 inspection. <br />Note that Morris Shaffer Enaineerina have since update their Waste Tank Svstem Intearitv Resort and MFS will <br />submit this update for SJC-EHD review and approval <br />Violation #104 - Failed to submit an onsite annual hazardous waste treatment notification by January 1 <br />R This violation was corrected ❑ This violation will be corrected by (date): <br />[& Supporting documents included <br />Describe actions taken or will be taken to correct violation: <br />The Facility submitted a letter "MFS intent to operate Stockton Facility Waste Treatment System (WTS) 2021" <br />on October 2, 2020 in the CERS <br />Violation #112 - Failed to prepare and implement a waste analysis plan and retain analytical results. <br />❑ This violation was corrected 0 This violation will be corrected by (date): <br />❑ Supporting documents included <br />October 2, 2021 <br />Describe actions taken or will be taken to correct violation: see below: <br />Per discussions with SJC-EHD on August 23, 2021, and correspondence with SJC-EHD Staff, MFS was <br />gathering information for which process tanks needed to be sampled and analyzed for the Waste Analysis Plan <br />.(WAP). During our August 23, 2021 discussion, SJC-EHS Staff directed all process tanks should be tested and <br />analyzed for the WAP, but subsequently gave updated information that only the rinse tanks (waters that are <br />treated by the PBR — WTS) should be analyzed. These tanks are 3,4,6,7,9,10 & 11. These tanks will be <br />analyzed for CAM 17 metals, fluoride and mercury using methods 208.1, 245 and 300.1, and will be incorporated <br />into the Facility's WAP, scheduled for completion by October 2, 2021 <br />Page 2 of 3 <br />Rev. 9/16/2020 <br />1868 E.
