FULL PERMIT: 3-hour minimum at the EHD's current hourly rate: <br /> An approved EHD permit is required before the start of the following work: <br /> 1. Underground storage tank(UST)system installation, expansion,or tank top upgrade <br /> 2. UST closure/removal <br /> 3. UST tank lining or repair <br /> 4. Regulated primary piping repair or replacement(downstream of the shear valve) <br /> 5. Secondary containment repairs including but not limited to piping, penetrations,sumps,and tanks <br /> 6. Direct bury spill container replacement <br /> 7. Overfill prevention installation, repair,or replacement <br /> 8. Dispenser conversion frames installation,dispenser replacement if conversion frame installation is <br /> required <br /> 9. Bravo float-and-chain removal(when used to satisfy monitoring requirements) <br /> All above repairs must have a full retrofit permit submitted to and approved by the Environmental Health <br /> Department(EHD) prior to beginning of work. If above components are repaired or replaced and the <br /> required permit was not obtained from the EHD,a full permit must be submitted to the EHD and a "Failure <br /> to Obtain a Permit” penalty(an additional 100%of the current permit fee)will apply. <br /> All repaired components must be tested within 30 days of repairs and test results must be submitted to the <br /> EHD within 30 days of testing.A 48-hour notification is required to be submitted to the EHD(via <br /> ust@s*cehd.com prior to testing). <br /> 2. PLAN CHECK SERVICE REQUEST: 1-hour minimum review time at <br /> the EHD's current hourly rate <br /> An approved EHD plan check service request is required before the start of the following work <br /> for non-emergency repairs or replacements: <br /> See the mandatory conditions below*: <br /> 1. Sensors(not"like for like") <br /> 2. Leak detectors(not"like for like") <br /> 3. Spill containers in tank top sumps(not"like for like") <br /> 4. Split retrofit penetration installations that do not require concrete breaking, piping disconnection,or <br /> sump/piping modification <br /> 5.
