Dispenser replacement due to structural damage(e,g.vehicular incident),without conversion frame <br /> installation <br /> 7. Sump lids repair/replacement for tanks installed after lune 30, 2003. <br /> All above repairs must have a plan check service request submitted to and approved by the EHD prior to <br /> beginning of work. If above components are repaired or replaced and the approved plan check service <br /> request is not obtained, a full permit must be submitted to the EHD and a"Failure to Obtain a Permit" <br /> penalty(an additional 100%of the current plan check service request fee)will apply. <br /> Emergency repairs relating only to the UST system monitoring and release detection, may be performed by <br /> a properly licensed,trained,and certified contractor as long as a complete plan check service request is <br /> submitted to the EHD within one business day of such emergency repair.A"Failure to Obtain a Permit" <br /> penalty(an additional 100%of the current plan check service request fee)will apply if the plan check service <br /> request is not submitted to the EHD within one business day.All required testing for emergency repair work <br /> must be scheduled within five days of the plan check service request approval. <br /> All repaired components must be tested within 30 days of repairs being made and test results must be <br /> submitted to the EHD within 30 days of testing.A 48-hour notification is required to be submitted to the <br /> EHD(via ust@sicehd.com prior to testing). <br /> *CONDITIONS FOR EHD PLAN CHECK ONLY REPAIRS <br /> This work requires EHD written approval prior to implementation: <br /> 1. Submit a Service Request form, a completed UST retrofit application form, pertinent cut sheets, <br /> all applicable technician's certifications, and a payment for 1 hour of review time at the EHD's <br /> current hourly rate <br /> 2. Once the application has been reviewed and approved, provide a 48-hour notification to the EHD <br /> of repair and testing by emailing to UST@sicehd.com or faxing to(209)468-3433. <br /> 3. The service company must install and test the repaired component(according to the <br /> manufacturer's installation instructions) to verify functionality. <br /> 4. The service company must provide the following documentation to the tank Owner/operator for <br /> inspector to review during annual UST inspection. <br /> a. Description of part(s) installed(parts list, cut sheets, etc.) <br /> b. Name& UST ICC#of Service technician performing the work (ICC, manufacturer <br /> certification proof, etc.,) <br /> c.
