Telephone: (209)948-6351 <br /> Program Element: 1600- FOOD PROGRAM Complaint#: C00053540 <br /> Responsible Party Contact: <br /> DESCRIPTION OF COMPLAINT ALLEGATIONS <br /> The San Joaquin County Environmental Health Department received the following complaint: <br /> COMPLAINANT ALLEGES THERE IS AN ACTIVE ROACH INFESTATION AT THIS FACILITY. <br /> VIOLATIONS AND CORRECTIVE ACTIONS <br /> Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br /> 113700.All violations must be corrected within specified timeframe. Violations that are classified as"MAJOR"pose an immediate threat to public health <br /> and have the potential to cause foodborne illness.All major violations must be corrected immediately.Non-compliance may warrant immediate closure of <br /> the food facility. <br /> OVERALL INSPECTION NOTES AND COMMENTS <br /> FOOD ITEM--LOCATION--TEMP°F--COMMENTS <br /> No Temperature Data Collected <br /> NOTES <br /> Complaint inspection. <br /> Upon arrival, I spoke with the manager on site in regards to the nature of the complaint.The manager stated that no one can <br /> enter the facility unless they are an associate and that no one has called to complain of any type of roach infestation on site. <br /> The manager continued to explained that the facility was recently service by a pest control company, but a receipt or <br /> documentation was not given for the service. I was able to contact the district manager,who stated that the facility is serviced <br /> on a monthly basis and would provide me with documentation of the services.While on site, I was able to observed two dead <br /> roaches, one near the ice machine and one underneath a shelving used for dry storage. No other pest or droppings were <br /> observed while on site.
