SAN JOAQUIN <br />COUNTY <br />Environmental Health Department <br />Underground Storage Tank Program Inspection Report <br />Facility Name: <br />Facility Address: <br />Date: <br />QUIK STOP MARKET #551121 <br />1196 W LOUISE AVE, MANTECA <br />May 19, 2021 <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />Item # <br />Remarks <br />203 <br />CCR 2712(b)(1-2) Failure to maintain monitoring records for 36 months and maintenance records for 60 months. <br />OBSERVATION: Maintenance and monitoring records for the last three years were not found on site. <br />- The 05/18/2020 overfill test results were not found on site for inspection. <br />- The 08/05/2019 secondary containment test results were not found on site for inspection. <br />- The 2019 Monitoring system certification test results were not found on site for inspection. <br />REGULATION GUIDANCE: These records shall be maintained on site for at least three years. Monitoring records <br />include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance <br />records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs <br />of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of <br />inventory readings and reconciliations. <br />CORRECTIVE ACTION: Immediately locate and maintain all missing maintenance and monitoring records for the <br />last three years on site and submit copies to the EHD. <br />This is a repeat violation, Class ll. <br />208 <br />CCR 2716(a -e) Failure to comply with the designated operator inspection requirements. <br />1) OBSERVATION: The designated operator failed to check and note all required items on the visual inspection <br />reports. The incorrect and missing information included the following from the inspection reports: <br />-The DO(designated operator) reports of 6/01/20, 06/29/2020, 09/01/2020,and 02/08/2021 the OWNER / <br />OPERATOR failed to provide a name, signature, and date within required time frame of receiving the DO report <br />under the ACKNOWLEDGMENT OF COMPLIANCE ISSUES section <br />- The DO report of 10/26/2020 did not include the owner/operator name. <br />REGULATION GUIDANCE: During the visual inspection, the designated operator shall check that all required <br />testing and maintenance for the UST system have been completed and shall check all required items on the report. <br />CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including <br />all of the required information on the reports. <br />2) OBSERVATION: The designated operator inspections for the following months were performed more than 30 <br />days after the previous inspection: <br />- Initial inspection on 7/23/2020, next inspection performed on 09/01/2020 (70 days) between the two dates. <br />REGULATION GUIDANCE: The designated operator visual inspections shall be conducted within 30 days or less of <br />the previous inspection. <br />CORRECTIVE ACTION: Ensure that all designated operator inspections are performed within the required time <br />frames. <br />This is a repeat violation, Class ll. <br />Overall Inspection Comments: <br />FA0000848 PR0231430 SCO01 05/19/2021 <br />EHD 23-01 Rev. 9/16/2020 Page 5 of 6 Underground Storage Tank Program OIR <br />1868 E.
