SAN J O A Q I I I N Environmental Health Department <br /> L -0 U N-1- Y I�I AMENDED <br /> 10/28/2019 <br /> RCRA Large Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> Dollar Tree Distribution Center-Stockton 1 1122 Runway Dr, Stockton October 14, 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 116 CCR 66262.42 Failed to comply with uniform hazardous waste manifest exception requirements. <br /> OBSERVATION: The facility did not have exception reports on site for the following Uniform Hazardous Waste <br /> Manifests(UHWM): <br /> -012963049FLE (5/3/2019)(see photos IMG 8999-9000) <br /> -012496413FLE (4/1/2019) <br /> -008553826FLE (8/30/2017) <br /> -008597674FLE (12/08/2016) <br /> -008664542FLE (11/23/2016)(IMG 8995) <br /> REGULATION GUIDANCE: <br /> (a)A generator who does not receive a copy of the manifest with the handwritten signature of the owner or operator <br /> of the designated facility within 35 days of the date the waste was accepted by the initial transporter shall contact <br /> the transporter and/or the owner or operator of the designated facility to determine the status of the hazardous <br /> waste. <br /> (b)A generator shall submit an Exception Report to the Department if the generator has not received a copy of the <br /> manifest with the handwritten signature of the owner or operator of the designated facility within 45 days of the date <br /> the waste was accepted by the initial transporter.The Exception Report shall include: <br /> (1)a legible copy of the manifest for which the generator does not have confirmation of delivery; <br /> (2)a cover letter signed by the generator or the generator's authorized representative explaining the efforts taken to <br /> locate the hazardous waste and the results of those efforts. <br /> (d)Generators shall submit the exception report to the department at: DTSC Report Repository, Generator <br /> Information Services Section, P.O. Box 806, Sacramento, CA 95812-0806 <br /> CORRECTIVE ACTION: Locate a copy of the destination facility signed manifests and send a copy to the San <br /> Joaquin County Environmental Health Department(EHD). If the destination facility signed manifests cannot be <br /> found then prepare an exception report for each missing manifest as required by Title 22 California Code of <br /> Regulations section 66262.42. The exception report shall include a legible copy of the manifest for which the <br /> generator does not have confirmation of delivery and a cover letter signed by the generator or the generator's <br /> authorized representative explaining the efforts taken to locate the hazardous waste and the results of those efforts. <br /> DTSC address is DTSC Report Repository, Generator Information Services Section, PO Box 806, Sacramento, CA <br /> 95812-0806.
