SAN J O A Q I I I N Environmental Health Department <br /> L -0 U N T Y I�I AMENDED <br /> 10/28/2019 <br /> RCRA Large Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> Dollar Tree Distribution Center-Stockton 1 1122 Runway Dr, Stockton October 14, 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 118 CCR 66265.16 Failed to maintain complete hazardous waste training records. <br /> OBSERVATION: <br /> A.The business operator could not produce documentation during the inspection that demonstrates employees <br /> have complete hazardous waste training as specified in this section.The following was missing from the training <br /> documentation (see photos IMG 9010-1): <br /> 1.There was no record of hazardous waste training for Rebecca Corona, Hazmat employee(IMG 9010-1). <br /> 2.The job title for each position at the facility related to hazardous waste management,and the name of the <br /> employee filling each job. <br /> 3.A written job description for each position listed under paragraph (d)(1)of this section. <br /> 4.A written description, including a syllabus and/or outline, of the type and amount of both introductory and <br /> continuing training that will be given to each person filling a position listed under paragraph (d)(1)of this section. <br /> B.At the time of inspection, it could not be demonstrated that employees who handle hazardous waste were <br /> properly trained, based on the following observations: <br /> 1. Five 741 oz bottles of Personal Care shave cream (UPC 808829114928)were found in a container labeled <br /> Hazardous Waste Aerosols, Flammables and Combustibles (AF)in the hazardous waste storage area(IMG <br /> 8670-3). However,when Rebecca scanned the shave cream,the hand-held scanner stated the shave cream <br /> should be disposed in the State Regulated, Toxics, OTC Pharmaceuticals&Universal Waste(STOU) Hazardous <br /> Waste tote(IMG 8674). <br /> 2. On 10/14/2019 an open unlabeled tote containing various potentially incompatible chemical containers,which <br /> were open and upside down,and appeared empty,was observed in the hazardous waste storage area (IMG 8649). <br /> A cloth was observed at the bottom of the tote, beneath the open containers. According to Sherry Aebersold, <br /> Environmental Health &Safety Compliance Manager, and Rebecca Corona, Hazmat employee,the chemical <br /> containers are opened and placed upside down in the tote to drain out any residual chemicals onto the cloth. The <br /> following chemical containers were observed inside the opened tote(IMG 8650-2): <br /> -A 16 fl oz bottle of Asssured 50% Isopropyl Alcohol <br /> -A 1 qt bottle of Diversey Crew Clinging Toilet Bowl Cleaner <br /> -A 56 fl oz bottle of LA's Totally Awesome Multi Surface Cleaner <br /> -An 8 fl oz bottle of Lysol Clean &Fresh Toilet Bowl Cleaner <br /> -A bottle of LA's Totally Awesome Carpet Spot&Stain Remover <br /> -Three 16.9 fl oz bottles of Fabuloso multi-use cleaners <br /> -A bottle of Ajax dish soap <br /> -A bottle of Palmolive dish soap <br /> -A bottle of Pine-Sol cleaner <br /> -A 1 qt, 8 oz bottle of Pine Glo cleaner <br /> -An unknown brand extra strength energy drink/shot <br /> According to Sherry and Rebecca, once drained,the empty containers are then transferred to a tote labeled "CA <br /> Cleaned and Empty"(IMG 8654).The empty containers observed in the"CA Cleaned and Empty"tote included, <br /> but were not limited to the following (IMG 8655-60): <br /> -Three bottles of Fabuloso multi-use cleaners <br /> -Two bottles of LA's Totally Awesome All-Purpose Concentrated Cleaner <br /> FA0013602 PR0539024 SCO01 10/14/2019 <br /> EHD 22-03 Rev.9/20/2019 Page 12 of 48 RCRA Large Quantity Hazardous Waste Generator OIR <br /> 1868 E.
