SAN J O A Q I I I N Environmental Health Department <br /> L -0 U N T Y I�I AMENDED <br /> 10/28/2019 <br /> RCRA Large Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> Dollar Tree Distribution Center-Stockton 1 1122 Runway Dr, Stockton October 14, 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> -A 96 oz bottle of LA's Totally Awesome Bleach <br /> -Two 24 fl oz bottle of Comet Classic Bleach Kitchen Cleaner <br /> -A bottle of Palmolive dish soap <br /> -A bottle of Pine-Sol cleaner <br /> -A bottle of Ajax dish soap <br /> -Two bottles of Kirkland Extra Strength Energy Shot <br /> -A 1 qt bottle of Diversey Crew Clinging Toilet Bowl Cleaner,which was labeled "Danger: Corrsoive"and <br /> "Chemical Hazards: Never Use With Chlorine Products...these can react to give chlorine gas." <br /> On 10/15/2019, the open unlabeled tote contained four 96 fl oz bleach containers and one bottle labeled"Paint <br /> Thinner,which were left open and upside down to drain out the residual chemicals from inside the containers(IMG <br /> 8917-9).There was no longer a cloth at the bottom of the tote, and it was unclear how the cloth observed on <br /> 10/14/2019 had been managed. <br /> 3. Charell Gaines, an employee,was observed sorting through product and damaged product at Pick Module 1 <br /> Zone A-B (IMG 8936-7, 8940-1).When asked what he would do with a 9 fl oz bottle of hand soap that was found <br /> damaged and leaking versus another undamaged bottle of the same hand soap, Charell stated he would throw the <br /> leaky damaged product in the trash and place the undamaged hand soap into a box for repackaging (IMG 8933-4). <br /> Charell proceeded to throw the leaky damaged bottle of hand soap into the blue dumpster next to his work area <br /> (IMG 8938-9). <br /> 4. The trash compactor was not locked and no one was actively placing trash into the compactor at the time of <br /> inspection of the compactor. Rebecca Corona, Hazmat employee,was asked by Sherry Aebersold, Environmental <br /> Health&Safety Compliance Manager, and Gustavo Garcia, Operations Manager,to demonstrate how trash is <br /> sorted before disposal into the compactor to ensure hazardous waste is not being disposed into the compactor. <br /> Rebecca dumped the trash from a blue dumpster into the trash compactor without sorting through the trash before <br /> disposal.When Sherry asked Rebecca when the sorting process had stopped, it was unclear by Rebecca's <br /> response when the sorting process had stopped.The leaky damaged bottle of hand soap that was previously <br /> thrown into the dumpster by Charell Gaines was found in the trash that had just been dumped into the compactor <br /> (IMG 8964-6).Two broken leaky 8 oz containers of Softee Extra Hold Protein Styling Gel were also found in the <br /> dumped trash in the compactor(IMG 8964, 8967-9). <br /> 5. The fallen, damaged, and/or leaking chemicals listed below were observed throughout the facility, on the floor, <br /> floor pallets, Pick Module(MOD)floor grates and concrete floor,facility equipment,or on product boxes in the <br /> MODS,without any facility personnel actively working to clean up and/or identify if these chemicals were hazardous <br /> waste.According to the Dollar Tree California (CA)Store Quick Reference Guide, damaged and leaking chemical <br /> items are to be identified as either waste, hazardous waste, or sellable product, and hazardous waste is to be <br /> managed/processed properly. It was unclear how often the facility cleans up and identifies fallen,damaged, and <br /> leaking chemicals throughout the facility. It was unknown how long ago the following containers of chemicals fell, <br /> became damaged, and/or began leaking: <br /> 1) Found on a floor pallet,an open unlabeled container of eleven bottles of orange dish soap, at least four of <br /> which were contaminated with a white substance,two which were open and partially empty, and at least one with a <br /> broken cap (IMG 8691-3).Two bottles of White Rain Body Wash, and a can of Rockstar Energy Drink were also <br /> found in the open container. <br /> FA0013602 PR0539024 SCO01 10/14/2019 <br /> EHD 22-03 Rev.9/20/2019 Page 13 of 48 RCRA Large Quantity Hazardous Waste Generator OIR <br /> 1868 E.
